How to Enter Institution Specific Questions (ISQs)

This article explains how to enter your Institution Specific Questions ISQs

Overview

Included with each Benchworks national order is the ability to add 20 questions created by the institution that appear at the end of the survey. These questions are called Institution Specific Questions (ISQs).

 

Login to MyEBI https://order.webebi.com/ClientSite/Login.aspx

 


 

From the "My Account" landing page, find the order for which you need to add ISQs and click on the "Task List" button.

 Then click the Institution Specific Questions (ISQ) link.


 



You may also access the ISQs from the Setup Task List.

 

If you are a returning customer, a pop up will appear allowing you to choose ISQs from previous order. Use the drop down to select the year from which you want the ISQs imported. To start from scratch click the Create New Institution Specific Questions button.

 

Choose the question type to be used. Please note that if you choose a question type and wish to change it, you will need to delete the question and re-add it with the new question type. Enter the question text and click “Save.” The system will indicate that the question has been saved.  Read more about question types here. 

 



Based on the question type selected, enter your answer options or select a scale. For example, if the question type selected is “Scaled,” then you need to choose a scale from the answers dropdown. You can choose a scale that has previously been created or create a new one. Check the box next to Include Not Applicable N/A Answer to add it to your scale.



 

For Categorical question types, choose the question type to be used (Drop Down or Radio Buttons).  Enter the question text on the left and click Save. The system will indicate that the question has been saved. On the right, enter the answers one by one in the Answer Text box and click the Add button for each. You may Edit, Delete or move answer options using the blue arrow icons.

 

You can return as many times as necessary to complete your ISQs. Once you’ve completed adding your questions click Save and Close.

 

Review all of your questions and answers before finalizing. Click the Lock ISQs button at the bottom right to finalize. If you need to make changes after they are locked, please contact support.
